00:04 - 00:06This is Brent of the Brookbush Institute,
00:06 - 00:07in this video we're going over
00:07 - 00:09the special or orthopedic test for the
00:09 - 00:12acromioclavicular joint, the crossbody or
00:12 - 00:14horizontal adduction test. I'm going to
00:14 - 00:15have my friend Melissa come out, she's
00:15 - 00:18going to help me demonstrate this test.
00:18 - 00:21Now this like our other AC joint test is
00:21 - 00:24a fairly simple test, all I'm going to do
00:24 - 00:26is start with Melissa 90 degrees of
00:26 - 00:27shoulder flexion, and then I'm going to
00:27 - 00:30passively take her into end range
00:30 - 00:33horizontal adduction, and give her a
00:33 - 00:35little over pressure. The next thing I'm
00:35 - 00:38going to ask is, is that the symptoms you
00:38 - 00:40were talking about? That was the was the
00:40 - 00:42pain you had huh, and as long as that
00:42 - 00:46pain is localized over the top of the AC-
00:46 - 00:49joint, then it's a positive test. Let me
00:49 - 00:51show you guys a couple other views here.
00:51 - 00:53So I'm going to have you turn forward
00:53 - 00:55here, so notice guysI get my elbows up
00:55 - 00:58nice and high so that I can not have to
00:58 - 01:00use my internal rotators to try to
01:00 - 01:01create some force; but I can use my
01:01 - 01:04stronger deltoids and PEC muscles and I
01:04 - 01:06is passively bring her in here, and you
01:06 - 01:08guys want to be kind of gentle with this
01:08 - 01:09motion, it's not comfortable for anybody.
01:09 - 01:12And then I'm going to push, give her a
01:12 - 01:15little over pressure. Is that the
01:15 - 01:17symptoms you were talking about? Alright
01:17 - 01:20i'm going to show you guys one more view.
01:20 - 01:24Here once again i used that same kind of
01:24 - 01:26brace that i used for a lot of these
01:26 - 01:29shoulder and AC joint special tests,
01:29 - 01:31which is my fingers curved over the top
01:31 - 01:33of the trap here, and then my palm over
01:33 - 01:35the back of the scapula, so I have nice
01:35 - 01:37control over scapula shoulder and torso.
01:37 - 01:39And then I'm going to use my other hand
01:39 - 01:43over her elbow, elbows nice and high, pull
01:43 - 01:45all the way into horizontal adduction,
01:45 - 01:48put a little over pressure. Is that the
01:48 - 01:50symptoms you're talking about? Alright
01:50 - 01:52let me have you go ahead and face this
01:52 - 01:56way. Now we do have a little bit of a
01:56 - 01:58problem with this test putting somebody
01:58 - 02:01in to complete horizontal adduction is
02:01 - 02:04likely to cause pain for anybody who has
02:04 - 02:05AC joint issues. It's also likely to
02:05 - 02:07cause pain in anybody who has shoulder
02:07 - 02:10impingement issues, so what can I do?Well
02:10 - 02:13I can cluster this test, which we're
02:13 - 02:16definitely going to cluster all of our
02:16 - 02:17special tests, we've been
02:17 - 02:18talking about that throughout all of our
02:18 - 02:21special testing videos. But let's say
02:21 - 02:27this was, she says yeah I have pain, well
02:27 - 02:29I could then follow up with palpation
02:29 - 02:31which is a special test unto itself,
02:31 - 02:34although not one that I would use by
02:34 - 02:37itself, but if I then palpated her AC-
02:37 - 02:41joint and I said, hey does that hurt, and
02:41 - 02:44she said yes; okay good I can keep going
02:44 - 02:46with my AC joint test. Maybe add a couple
02:46 - 02:49more of these special tests, create an AC
02:49 - 02:51joint cluster, and that will give me a
02:51 - 02:53better indication of what's going on
02:53 - 02:55with her AC joint. If she said no when I
02:55 - 02:58was palpating, alright so go ahead and no
02:58 - 03:01no okay if I said no, palpation is
03:01 - 03:03actually very sensitive, not very
03:03 - 03:06specific but very sensitive, so a
03:06 - 03:08negative test result is probably a
03:08 - 03:12pretty good clearance that things are
03:12 - 03:14not happening at the AC joint. And then
03:14 - 03:17rather than going down the AC joint
03:17 - 03:19cluster, maybe I should go down my
03:19 - 03:22shoulder impingement cluster. Alright so
03:22 - 03:24just to review that one more time if she
03:24 - 03:28was positive for this test in that it
03:28 - 03:32gave her pain, and then I pushed down on
03:32 - 03:35her AC joint and she said no, I would
03:35 - 03:36probably go down the shoulder
03:36 - 03:41impingement path. If she said yes I go
03:41 - 03:43down my AC joint path and add a couple
03:43 - 03:48more AC joint tests. Thank you Melissa. So
03:48 - 03:50there you guys have it another test to
03:50 - 03:54add to your toolbox. I hope by now with
03:54 - 03:55all the special tests that we've gone
03:55 - 03:57through you guys are starting to get a
03:57 - 04:01good idea of not only how to use or
04:01 - 04:03what these special tests are, a good
04:03 - 04:04repertoire of special tests, but how to
04:04 - 04:07use them, how to cluster them to build a
04:07 - 04:10case towards accurate diagnosis. I look
