Tibial Internal Rotator Activation Taping

This video will teach you the technique of taping to initially activate and then protect the tibial internal rotators. A simple, fast, and effective way to help strengthen knee stability and improve ankle mobility.

00:06 - 00:09This is Brent of the Brookbush Institute at the Independent Training Spot, doing
00:09 - 00:15tibial internal rotator taping. So generally i use taping to reinforce
00:15 - 00:20activation techniques done within the session. This particular taping is the
00:20 - 00:25combination of VMO and popliteus activation tapings, to reinforce that
00:25 - 00:30tibial internal rotator activation exercise which I created a video for
00:30 - 00:35previously. Now generally the individuals who would receive this technique, or
00:35 - 00:39receive this taping at the end of their session, are those individuals who maybe
00:39 - 00:43had feet turn out, or knees bow in in their overhead squat assessment, a lot of
00:43 - 00:50those individuals come in and they have either ankle, knee, or hip pathology. This
00:50 - 00:54particular technique seems to also be very effective for those individuals who
00:54 - 00:58have knee pain, whether it's anterior knee pain, medial knee pain, or lateral
00:58 - 01:04knee pain. I think just having the tape around the knee creates a little bit of
01:04 - 01:12maybe distraction, maybe extra proprioceptive input that helps reduce
01:12 - 01:15the sensation of pain. I'm going to have my friend Jordan come out, he's going to
01:15 - 01:23help me demonstrate this technique. Now I went ahead and pre-cut my strips here,
01:23 - 01:28you're going to need a foam roll or a dowel, because both of these tapings are
01:28 - 01:32done with just a little bit of knee flexion. If you don't use a little bit of
01:32 - 01:36knee flexion, actually as soon as Jordan would go into let's say full knee
01:36 - 01:40flexion, like went down into a kneeling position, the tape would rip right off. So
01:40 - 01:44first thing we're going to start with is this VMO taping. I'm going to go ahead
01:44 - 01:49and pull his shorts up a little bit to about mid-thigh. Now for Jordan i went
01:49 - 01:55ahead and i cut a strip to about seven boxes long. Now for most individuals
01:55 - 02:01Jordan's fairly tall, i think five or six boxes will be sufficient. You guys know
02:01 - 02:05that I generally don't like to put a whole lot of tension in the tape, as I'm
02:05 - 02:09stretching the tape, or applying the tape, I would rather put the muscle in as
02:09 - 02:15short a position as possible, and allow just the return to neutral position to
02:15 - 02:18create the tension within the tape. I think that's just a little bit more consistent.
02:18 - 02:24It also doesn't allow for so much tension in the tape, that the tape pulls
02:24 - 02:29itself off. You guys can see here I've rounded my corners. I'm going to go ahead
02:29 - 02:38and take about three centimeters and rip the paper off, so that I have my anchor. The
02:38 - 02:42tricky one about the VMO taping is, number one getting somebody to hold that
02:42 - 02:47foot in, so that they have that tibial internal rotation, and then making sure
02:47 - 02:51you have the angle right. If you start like this and try to stretch the tape
02:51 - 02:56around, you're going to end up with little wrinkles in the tape. But if you
02:56 - 03:02kind of measure out where you want to go, and then apply your anchor at that angle,
03:02 - 03:07and what you guys can see I'm doing here is my anchor is right over his tibial
03:07 - 03:15tuberosity. I like to to get the tibial tuberosity in there as the insertion of
03:15 - 03:21the vastus medialis oblique. I'm going to make sure he's turned in and then I'm
03:21 - 03:26going to go ahead and use tape off tension along his VMO, and remember
03:26 - 03:30your VMO is a fairly long muscle, despite the fact that we call it the teardrop
03:30 - 03:35muscle and we only think of it being this long. The VMO goes all the way up
03:35 - 03:43into those adductor tendons. Alright make sure we give a little friction rub
03:43 - 03:51here, Heat up that adhesive. The adhesive is heat activated. Once i'm pretty sure
03:51 - 03:55that i got that nice and stuck on, i'll go ahead and pull down his shorts over
03:55 - 03:58the tape so that he doesn't catch the end, and then i'm going to have Jordan go
03:58 - 04:10ahead and turn over. Now instead of having the foam roll or bolster under
04:10 - 04:12his knee to create that little bit of knee flexion, I'm just going to have it
04:12 - 04:21underneath his ankles. Now for the popliteus taping, you're actually going
04:21 - 04:26to cut a little shorter tape. So this is about one, two, three, four, a little over
04:26 - 04:31five squares, might be a little smaller for shorter people. You're going to bring
04:31 - 04:34this foot in as far as you can, he's in a little bit of knee flexion,
04:34 - 04:39that will create the shortening. Alright and the popliteus tape I actually
04:39 - 04:48find works easier if you tear it in the middle, peel back like so, so now you take
04:48 - 04:58the tape and the paper here, and you pull it back so that you're going to lay the
04:58 - 05:03middle of the tape just over where you think the popliteus would be. So over the
05:03 - 05:09joint line, kind of with the anchors being over the medial gastroc, and right
05:09 - 05:15around the iliotibial band there. So I'm going to go ahead and put that down like
05:15 - 05:31this. I'm just going to go ahead and tape off their, paper off rather, cool. So now
05:31 - 05:36you can see this kind of goes from lateral to medial, superior to inferior,
05:36 - 05:43and what I've actually done even if we forget what muscles are actually playing
05:43 - 05:50a role in tibial internal rotation, is I've given him this little cue to keep his
05:50 - 05:56tibial from excessively externally rotating. Jordan you mind standing up on
05:56 - 05:58the table here.
06:01 - 06:06The reason i like this tape or this particular taping technique, besides
06:06 - 06:09the fact that people with knee pain generally feel really good when they put
06:09 - 06:14it on, is as soon as he went into extension, where do you feel a whole lot
06:14 - 06:20of tension, the back tape right, yeah. So he goes into full extension, he's got the
06:20 - 06:24popliteus tape pulling him into tibial internal rotation, and when he goes into
06:24 - 06:30flexion the VMO tape is pulling him into tibial internal rotation. So that's that
06:30 - 06:36constant little reminder to pull him out of, go ahead and show an excessive feet
06:36 - 06:41turn out, can you do that right. This is what we don't want to see. We don't want
06:41 - 06:45to see his feet just splay out, so even as he's walking I know he's
06:45 - 06:50demonstrating a squat now, even as he's walking he's getting that little signal
06:50 - 06:57to keep tibia in line and not excessively externally rotate. If I just
06:57 - 07:02did all of that activation work, and let's say I even did my release,
07:02 - 07:09stretching, mobilization, activation, and integration work. I now have this taping
07:09 - 07:16working for me as his therapist for a couple days hopefully, maybe even more.
07:16 - 07:20If he's very careful about not catching this tape with the edge of his
07:20 - 07:25clothing, this might even last three four or five days. That's a constant little
07:25 - 07:30reminder that's working for me as his therapist until he gets back for his
07:30 - 07:36next session, and as I mentioned before guys the carryover I've had with tape
07:36 - 07:41has been absolutely incredible. Thank you Jordan, guys I hope you have wonderful

07:46 - 07:51it's going to give you a new tool on trying to fix that feet turn out problem.
