This is Brent of Brookbush Institute
00:00:0500:00:07
in this video we're going over another
00:00:0700:00:11
special or orthopedic tests for sacroiliac joint pain, this is the
00:00:1100:00:14
Mennell's test. I'm going to have my friend Melissa step out she's going to help me
00:00:1400:00:18
demonstrate. Now the way we're going to set this up is Melissa is going to be supine,
00:00:1800:00:22
with the sacroiliac joint I think that's involved, closer to me. Now this test
00:00:2200:00:26
isn't very hard to perform it is kind of tough on your patient though, you'll see
00:00:2600:00:30
why in a second, I have to to hold her leg in kind of a difficult position. The test
00:00:3000:00:33
itself is just Melissa's going to bring herself into ten degrees of flexion,
00:00:3300:00:37
thirty degrees of abduction and then she has to be able to hold that, which is
00:00:3700:00:44
tough, and then she has to resist some extension force and some force into
00:00:4400:00:50
flexion, and we're going see if that brings on her concordant sign or her
00:00:5000:00:55
complaints of sacroiliac joint pain. So one more time I mean that that's the
00:00:5500:00:59
whole test, that's it, it's not that hard. So she's going to go into ten
00:00:5900:01:03
degrees of flexion, thirty degrees of abduction and I'm actually pushing down
00:01:0300:01:09
and then up in the sagittal plane. Alright so down how does that feel? Pain,
00:01:0900:01:15
and then up, okay good how does that feel? Fine okay.
00:01:1500:01:20
Here's the problem with this test though, when you felt pain where did you feel it?
00:01:2000:01:26
So if she feels it in her anterior hip, okay not her pubis but her anterior hip,
00:01:2600:01:32
what do you guys think the likelihood of this being sacroiliac joint pain is?
00:01:3200:01:37
Probably not very good right, like we were pushing down as she was trying to
00:01:3700:01:43
lift up, she probably felt it more like in her psoas or maybe her TFL as it
00:01:4300:01:48
cramped to try to pull her leg back up against my force. I wouldn't call that a
00:01:4800:01:53
positive sign, and that is one of the bigger issues with this particular
00:01:5300:01:59
test is if anybody has any hip issues this probably isn't a good test to use,
00:01:5900:02:06
but again this test ends up in one of our SIJ clusters, not the one that we
00:02:0600:02:09
prefer here at the Brookbush Institute but one of the sacroiliac joint clusters
00:02:0900:02:13
does use the Mennell's test as one of the three or five tests options
00:02:1300:02:18
in this cluster. So it is important for you guys to have in your repertoire so
00:02:1800:02:22
that you're aware of this test and if you're going to use it in that cluster
00:02:2200:02:25
you've got to be pretty proficient at it. So again let's just kind of go through
00:02:2500:02:32
the test itself and contraindications. Hip flexion, thirty degrees of abduction,
00:02:3200:02:39
Melissa hold this okay don't let me push you down, alright how does that feel? Okay
00:02:3900:02:44
but it's still pain here right, and then go don't let me pull you up, good
00:02:4400:02:48
how does that feel? That feels fine, so that's definitely a negative
00:02:4800:02:52
Mennell's. Now if I pushed down or pushed up and she felt it back in in that
00:02:5200:02:58
sacroiliac joint region, that would definitely be a positive test. We could
00:02:5800:03:02
probably make a good argument that if she felt it in her pubis right that that
00:03:0200:03:07
still would be a positive test, because we know we have some sort of instability
00:03:0700:03:11
of the pelvic ring there. Thank You Melissa. Guys if you have any questions
00:03:1100:03:16
please leave them in the comments box below, I look forward to hearing from you
